Dental implant rejection

I had an implant put in 2 months ago and it came out. I was told this is 'rejection' but I'm not sure what that means. Like, why would my mouth reject an implant?

I have to go back and see my dentist about this... but am I gonna have to pay for a whole new implant? Whose fault is it that it came out? Mine or the doctor's?
