Dental Implants Faling Out - WHY ?

I have had two dental implants fall out. My dentist said with the first one that the bone had completely deteriorated under the implant.

The 2nd one just fell out this week and I haven't contacted my dentist yet (because of the holidays). Why did this happen?

Any chance you think I could get a refund for these two implants?

Implant screw has fallen out

How can I tell if the implant is still there,I see a white small patch that looks like bone not sure if that is the implant.
